The Head of Service for Health and Wellbeing, HSE Midlands Louth Meath
CHO (CHO8) ? Meath, is inviting expressions of interest from
organisations to deliver a Community Connector service by employing
and managing a community connector post for Older Persons. The
Community Connector (Older Persons) will operate across 3 Community
Health Networks (CHNs) in the Meath area: North Meath Ardee (including
Kingscourt), Central Meath and Southwest of Meath.
Successful organisations will be contracted by HSE via Section **Apply

to deliver a specified service.
